Self Assessment Tools

It is important to note that self-assessments should be reviewed with a licensed mental health provider. Some questions and results are designed for clinical interpretation and can be easily miscontrued. If you feel medication can be helpful to your treatment, you can contact your primary care provider for a referral to psychiatry. Pathfinder is available to assist with referrals and coordination of care for the best outcomes.
